Describe the study area
Using features such as roads, rivers, or city limits as the boundaries, or by noting how much area to include in any given direction from a center point, describe an area around your church, or site, you wish to study. Or send a map showing that area. (Your description will be converted to Census Tracts and assembled into your study area. Hint: Keep the area as small as possible).

Tell us about your church      
Your study will have 12 maps showing demographic and church data. If you are an existing church, we need your full street address (and city, state and zipcode) in order to pinpoint it on the maps. If a new church, provide the address(es) of your denomination's nearby church(Es).
The study also incorporates data on your church, if the report is for an existing church, a relocation or a renewal. We do retain this information in our database, but use it only for mapping purposes.

For your committee, you can order the Study Packet -- a workbook, 6 LPs copies and 6 color transparencies for $129.50, plus $12.50 shipping. Delivery is by two-day priority mail. Allow three weeks for preparation and delivery.
